What are the reasons for the failures in Information and Technology Projects?

when you make the project for the people thinking inside the box not outside the box.(its main make your project freedom and make your project characteristic).

Awarness and we need  to follow rules and regulation  

Poor in deploying an Assessment and Authorization (A&A) framework system. Keep in mind that the best A&A system is documented in NIST SP800-37 which called Risk Management Framework (RMF)

After spending years in both IT & project managemnet in the ME , I can say the following

1- there is not enough understading of project managemnet discipline among users, management and vendors.

2- everyone looks at the PM as the sole engine for a project , not true the engine consists of the accumulative power of its members, this is an accountability problem and resources assignment and PM not having enough influence on members and senior managemnet.

3- scoping, scoping , scoping

4- estimates, estimates, estimates in a scientific manner and not subjective.
