In Larger projects there are teams like CAB (Change Advisory Board and /or CCB (Change Control Board) to track and manage Change Management. C M should be consider during planning of any changes and before the any action on that planning. In planning Why, When, How, and what the impact after changes.
Some companies handle the change management by establish a Change Control Board "CCB" which are responsible for approve or deny any kind of changes in the projects
The results subsequent to the implementation of change are the only measure of change management.
