I try to be objective when I look at a problem, step outside of myself and my preconceptions. I talk to everyone. They'll all have either evidence or an opinion, or both. Review any written material in relation to the problem. Estimate the scope of the issue and find a solution acceptable to as many affected people as possible.
A complete research process is required before making a final decision. Effective problem solving steps are:
First, you have to define the problem
If you have defined the problem, you can start with solving the problem
In My opinion steps that i should follow are :
1 - Understand the problem
2 - Define the end result : what does success means ?
3 - Collect information
4 - Define the work to be done
5 - Plan the work
6 - Do the work
7 - Check and review
To study & solve a problem I will go straight for these steps:
1: Understand the nature of problem.
2: Causes
3: Responsible person or item
4: Evaluate the problem according our work strategies.
5: What reactions will be produce by that problem?
6: Calculate the money, time, progress loss to cover the difference.
7: Investigate those events which create this problematic act.
8: Feel myself in that situation to understand more better the impacts.
9: Check all the monitoring people for the responsibilities.
10: Point out the real root cause.
11: Suggest the best immediate solution which is acceptable.
12: Study & discuss the best solution.
13: Bring my solutions in the front of owner, client affected groups to go for the acceptable solution with minimum loss.
14: Get the approval to follow and implement the solution.
15: Plan, execute and follow the monitoring procedure.
16: Check & balance and proper control to block problems for future.
17: Suggest, approve and implement the punishment for irresponsible human involvement.
18: Make my final report, print and keep as documented shape for present & future staff to read and get guideline.

Actually it depends on the problem, its size, its impact, its public relatedness, its urgency and how much time we have to solve, available resources, the desirable strategy for action and if the intervention will be for the short term, or the long term, or is it temporarily or permanently; and also this will depend on the situations. But in general, we may:
- Prepare (or assemble in case it is already prepared) the team that is responsible for the intervention, including all involved stakeholders.
- Study the problem itself, its nature, its essence
- Assess the impact, the situation, and the risks (currently and within various time frames)
- Assess the opportunities that may be underneath & covered by the problem. Every problen has its good side too; i.e. opportunities including the opportunities to make things better for the next time
- Analyse the causes at several levels, using some tools like5-Why(s), to reach as deep and much as causing factors, so that when we plan for a solution we will not miss any important factors
- Make sure that we understand what mentioned above very clearly, and we can test our results to finalize the assessment phase.
- Communicate the assessment results with the main stakeholders, and coordinate properly so that the solution can be planned & initiated
- Develop our experience & learning so that we will be more immune to the same or similar problems in the future.
well i think it is dependent upon the right information being available to the right people at the right times.
Anyways i would follow these steps.

2.Identify limiting factors.
3.Develop potential alternatives.
4.Analyze the alternatives.
5.Select the best alternative.
6.Implement the decision.
7.Establish a control and evaluation system.
