Near Miss is unplanned, unwanted event that had potential to lead to injury, damage or loss.
To prevent a near miss, one must:1.
Take Near Miss as Serious as actual incident.2.
Do proper investigation and indentify immediate and Root Causes.3.
Identify suitable control measures4.
Then plan the remidial actions with Responsible person and time scale & Priority.5.

Well, i dont think this should be the question because, a near miss is treated like any other type of incident , the only difference is that the near miss doesn,t have an actual impact on human(regarding injuries) but it does have a potential to harm, injure or cause equipment damage, so the real question is how can we prevent an incident on work location? Well, it doesn't matter if you believe in fate and it's according to Gods well(which is a fact for me) however safety is a culture of community, a team performance, not an individual attitude, it depends on experience, acting, courses (on job and off job) hazard hunt on worksite, risk assessment for every job calling TOFS when required or even in doubt to reassess situation , management of change, cross reference between jobs and jobs locations within the same worksite All these factors combine together to form a safety performance that is translated throughout the year into KPI(Key performance indicator) that evaluates the performance within operation and safety and statistics can show what went wrong and how can we avoid the same mistakes for the next year One final word, Most of safety rules and figures(like minimum height of6 feet for using fall protection) were written with blood Work Safe everyBody

Near Miss: Any form of event which could have resulted in injury or loss but did not in fact do so– but did not actualize due to chance, corrective action and/or timely intervention.
Employees may be reluctant to report near misses due to potential recriminations that could result.
Such as:  Peer pressure  Disciplinary action  Unintended disciplinary action.
For example, upon incident investigation, additional job tasks or wearing cumbersome PPE may be perceived as punishment for reporting Objective of Near Miss:  Identify risks and prevent future accidents  Fix problems as they are identified  Make everyone an advocate of safety  Increase safety awareness.
The following eight steps should be done to have an Effective Near Miss Program.1.

Resolution Conclusion: The more near misses are reported, the less near misses would happened in the work place where the above is been understood and implemented.

Most of the workers does not know what a near miss is..
and hence they often never report it....
even if they know what it is, most of them never bother to report the same cos that 'll delay their work, or they feel they might land in trouble cos they might be held responsible for the near miss..
I think we should first train a worker on what is A NEAR MISS and the IMPORTANCE of report it..
then most of the safety officers act as Police men or as Inspectors rather than human beings..
If a positive attitude is cultivated, workers will automatically come up with reports of near miss...
then I totally agree with SHARIQ MUSHTAQ, HSEQ Engineer at QVC on what to be done.

Making Hazard Recognition, Evaluation and control a part of routine activity.
this has to become a part of the culture of the organization.
Also supporting a culture of reporting (hazards and near misses) free from reprisals.
