Is there a way to bring it to their attention anonymously? This is your decision to approach them directly and in a non-threatening way, point out what was done wrong and offer a solution.
This is a difficult question to answer because it depends on your status within the company. If you are not an equal peer of the boss/manager, they might feel intimidated by you and the possibility that your knowledge is equal or greater. Hopefully they will be thankful that a mistake was pointed out and how it can be prevented in the future.

According to professinal ethics, I believed that you must inform him immediatly , noting that if such matter is crucial it will have a negative performance upon Organization, but you should choice right time, & using professional debate to express such issue on a positive & professional way.
First of all no boss will be their who is100% wrong(At least80% he is right).Boss who acquired his position after long years of his hard work and experience.
Anyway it is better to discuss with in the department and approach your Boss when he is in tense free mood. Discuss the after effects of that particular issue in a polite manner and don't talk like Boss is always wrong i am right. If he is not ready to hear you then you can go to higher authorities.

بحكم انك تعرفه جيدا عليك بإخباره بتلك الطريقة اللتي هو يتقبلها دائما منك (( مثلا عن طريق مزحه او فكاهه او بطريقة وديه )) ثم تجنب ان تمارس دور او شخصية انك أخبر أو أعلم منه لكي لاتكسر كبرياءه الشخصي .. هذا فقط رأيي الشخصي ومن خلال تجربه
1. Listen to the boss politely until he has finished.2. Ask him if he has finished with his comments, then ask if ok for you to speak.3. Then give your answer, politely but confidently and even better with supporting proof/evidence if you have it.4. Be prepared for the boss to interrupt you - the boss most definitely will. Do not be put off. Leave a silent pause after the interruption and continue.5. Speak until you have completed your answer. The boss will either let the ego get in the way and not listen or be quiet and humble the self and take in your words.6. As long as you are sure that you have done the best that you could with your answer, leave evidence/documentation with the boss and say you'll leave the boss to ponder over it.7. The boss may still not listen but the important thing is that you have professionally given your account of another view which respectfully needs to be considered, especially if you could be benefiting the business.
I do not say her/him that s/he is100% wrong but I will explain the situation, condition s/he is explaining/doing is not the correct way to benefit the organization. I will explain it will harm to organization and its fame, economy, customer etc. Then, they will be convinced that s/he is100% wrong without speaking it directly.
لا يوجد احد خالى من الاخطاء حتى المدير يخطىء المشكله فى تكرار الخطىء او الاصرار والعناد على شىء يبدوا واضح انه خطىء
- واذا علمت ان المدير مخطىء احاول اعانته على تصحيح الخطىء بأسلوب راقى بحيث الا اتعدى حدودى الوظيفيه
والتعامل كأن اذكره مثلا بأن العمل جيد لاكن يوجد بعض الملاحظات الوارده فى اى نظام
لاكن اذا استمر المدير فى الاخطاء التى من شئنها ان تجهدنى او تأتى بعواقب سيئه على العمل مع اصرار المدير على ذالك ينبغى دراسه الوضع الحالى والتفكير فى عمل بديل
Well at first step I would simply follow the British work rule #9 &10 :).
Rule number9 is : "Boss is always right".
Rule number10 is : "if any time he is wrong, see rule number9". :)
But later on by my soft gestures, results of the same task on which my boss is wrong and with the simple sense of humor I would prove him that he also can be wrong any time, so he/she should not present him/her100% right every time. I am sure the problem will be eliminated forever.

As one of the top executives in the company, it would be my job to keep the current company affairs and future growth in perspective. If I found my boss making the wrong decision, my job would be to professionally bring the better ideas to his knowledge. As a loyal company employee, however, it would be my responsibility to see that company sails out unhurt amid the boss' unwise decision.
