it is a method that established in 1960s to find out any defect in the system by detailing the subsystem deficiacy. Usually it applied to investigate for equpments incidents as the manufacture industry is growing rapidely during that years. Thus in my opinion, It is not suitable to investigate incidents that involving human factor as the human factor study itself just starting on 1970s
