How to make discrepancy report for unacceptable or rejected items?

Please be informed that on18th of August,as we were inspecting our storage rooms we found that some of our items were rejects .
Below is the description of items that we found in our warehouse Cooking Oil Cheese Corn flour   As per the health protocals we will be writing to the manufacterers to validate all this items since they not reached the expiry date Yours S.S.O Daniel
