I was speaking to a ISO Consultant and ask him personally if employee survey does really have the necessary questions to get the opinion of the employees. He showed me a sample questionnaire and to my surprise, Yes it has all the questions from management to single unit of the company. But the thing is even if the questions are ready to answered by employees could not answer it with all their idean in mind. Survey is just there but, but no matter what happen to that survey will never ever make some changes to the company to look for their own improvements.
Stop guessing, and ask employees if they’re happy on the job. Use employee surveys to check in with employees about their satisfaction with their roles and responsibilities

the survey itself is rarely the problem. The questions are usually quite good in terms of defining the information you and the organization want to learn about employee attitudes, satisfaction and engagement.
