Many of the Organizations think that money is the main motivating factor, there are other areas which could be dominating, work culture, work ethics, stress and employee crediblity, one should try and research about the constrains that are affecting a healthy working environment, many from the senior staff leave because they feel they don't need to be here and junior because they are often get been there done that attitude, I would suggest to treat this problem in the long run instead of letting the employee filling out a form its better that a personel not the manager, should speak to him this might not help retaining him but definately have its impact in the long run
