In terms of expertise every country produce competent ESL teachers. I don't think of a monopoly of a country in this field.
In terms of earning China, South Korea, Middle East are considered better for ESL teachers.
China is known as the most powrful country in many fields as in ESL teachers .
Teaching English as second a language has grown exponentially. It is unfortunate to see incompetent or unqualified ESL teachers in schools and language centers. As mentioned in previous replies, we cannot discern the countries that produce 'above-standard' teachers. Moreover, you can find many native English speakers who are ESL teachers but struggle understanding language functions, parts of speech, grammar...etc.
It all comes down to the teachers themselves in terms of professional integrity, self-learning and commitment.
I feel the sub-continent, India and Pakistan have most English Language Teachers as it is their second language and they are developing a lot.
Country has no influence in teaching rather than it depends on the caliber of the teacher.A person from any country can be an excellent teacher with dedication and by using proper teaching strategies and techniques.
I would not generalize it by country. It takes only six seconds for students to know if the teacher is good or not. It depends on the technique used!
I think former colonies of English speaking countries have adopted both accents and better understanding of the English Language. In addition to the above assertion is the would be the education systems most countries with English as a teaching delivery language seem to produce a work force that is generally impeccable at both the understanding and teaching of English. I cannot attest to other countries' abilities of ESL teaching because i lack the exposure but i would say Zimbabwean educators are equipped to provide ESL teaching.
