Nonsense. The boss is sometimes wrong and sometimes right. As an employee your ultimate duty is to the shareholders, not your boss, so when the boss is wrong you have an obligation to do something.
It is always a sensitive matter when somebody believes they are right when they are in fact wrong. It is extra sensitive when it is your boss, and it is important not to undermine her/his authority.

The answer entirely depends upon the size, environment and nature of business/company. Multinational companies normally adopt open door policy; every person can express his views freely irrespective of his/her designation. But in certain institutions (e.g. Police, Army), orders from seniors are taken very seriously and fulfilled without a second thought.

I do not believe this phrase as it might be came up in those old days when very few experienced personnel were available. Nowadays knowledge & experience are widely available in any industries and sharing of knowledge & collective work is also high impact on the decision making. One should definitely acknowledge & respect to the status of boss but should not indulge in stating such phrases to downgrade our own respect.
Rubbish! The boss cannot possibly be right all the time.
The employee has equal allegiance and commitement to the organization as a whole as s/he does to the boss, so if the boss's judgment is questionable then the matter should be adressed.
The nature of the matter is delicate to say the least, so find a balance of words that do not undermine the boss's authority.
