Everything is a chemistry between Trust, Results, Approval and Communication between The Manager and the Client/Consultant
When the results are not being realized as expected, client/consultant has to identify the reasons. In rare cases project manager and in most of the cases one or a couple of other key personnel (mostly in consultation with the project manager) are identified as the reason and are replaced.
As regards the identification process, I agree to half of the Mr Zafarullah Shareif's diagnoses: Trust. Anyone who can't win over the trust of the client would always be in danger of replacement. Whether that trust of mistrust is based on merit depends upon the knowledge and efficiency of the client/consultant.
Relationship between the Project Manager and Customers/Clients are mainly grounded on TRUST and TRANSPARENT dealings; if this is not attained there will be always fear of gaps between them. Any kind of complications come across in the project PM should communicate to all of its stakeholder on time specially he has to be focus on clients and consider their behavior; If he loose Trust then PM existence will be in "endanger".
I agree with all answers, its a matter of trust, most of cases the client and consultant trying to find a solution for a major issue by changing the PM or key player in the project instead of fixing the relation or finding the weak points and giving some support which i believe its less costly and more safely for the project.
