i am using and practicing Prosci for the change management model.

the phases as below:

Phase 1 – Preparing for Change

Phase 2 – Managing Change

Phase 3 – Reinforcing Change

all  of the above phases build to follow the ADKAR model represented by:

1- AWARENESS of the need for change

2- DESIRE to participate and support the change

3- KNOWLEDGE on how to change

4- ABILITY to implement required skills and behaviors

5- REINFORCEMENT to sustain the change

if any info needed feel free to submit it here

Either Prosci or Kurt Lewis it is a three step change journey process:

1. Identifying and preparation for the destination - maps/ where

2. Execution of the steps - driving

3. Monitoring the right path to reach the destination - checking back with the map

Hope this helps!

Lemins model is the simplest

But Kotters 8 step change model is more definitive

Create Urgency

Form a Powerful Coalition

Create a Vision for Change

Communicate the Vision

Remove Obstacles

Create Short-Term Wins

Build on the Change

Anchor the Changes in Corporate Culture
