The single common factor with regard to HSE is people. Processes, equipment, regulations etc all require people to implement and deliver. Lack of understanding, poor culture, insufficient leadership, etc. etc. lead to HSE mistakes.
Implementing an HSE plan is easy, delivering against it, is not. You can put the rules, process etc in place, but if people do not follow these the plan fails and more often than not, something happens.
The reason is that Higher mangement does not give 1st priorty to HSE.
The reasons could be many, but HSE sould be taken very serious and the failure to its implementation obeys to the negligence of the planning manager or of the owner of the project...
the major causes of HSE plan failure include lack of training, team involvement in applying the plan since they are not convinced with the importance of HSE importance
some times also the HSE plan itself is not practical to be applied since it conflict and/or delay execution plans from the team point of view so they ignore it

Almost all the organizations talk big about HSE, yet not much of an importance is attached to HSE when it comes to implementing.
