Responsibility and Accountability - Do they go hand in hand?

Of course!

I am accountable for the result of the idea generated, while I am responsible for my team's success in transforming the idea generated into the result expected. Both goes hand in hand as working together to accomplish.

Responsibility  (taken form official site) means that you are involved, possibly performing a clearly defined task, and your performance could determine a successful outcome.

But accountability tend to person him-herself , he-she has a right to choose to ensure that there will be a successful outcome.

So YES i think so , they can be hand in hand .......
