Hi,

Hazards and Operability (HAZOP) study employed a structured technique to identify (i) hazards

that could lead to consequences of significance and (ii) approaches for reducing the risk of events

associated with the identified hazards. The process engineer, instrument engineer, mechanical &electrical engineer, operators participate in this study.

The scope and objectives of the HAZOP are to:

Identify hazards that could lead to consequences of significance. For this analysis,

consequences of significance were limited to accidents and/or abnormal/undesired events

(e.g. hazardous material releases) that could lead to:

a. serious injury to people,

b. major equipment damage (assets),

c. imminent and/or substantial endangerment to the environment,

d. production loss,

e. reputation impact.

The review also included evaluating routine and non-routine phases of operation (including

startup, shutdown, maintenance, and sampling) for potential equipment failures, human

errors, and process problems that could result in consequences of significance.

Consequences not considered of interest included those resulting from routine occupational

hazards (e.g. employee slips, trips, falls…etc.). Events that were determined to be

probabilistically implausible, during normal conditions, were not evaluated.

A Hazard and Operability Study systematically investigates each element in a process. The goal is to find potential situations that would cause that element to pose a hazard or limit the operability of the process as a whole. 4-Steps to carry out HAZOP Studies: a). HAZOP team formation; b). System element identification; c).Operations parameters variation possibilities; d). Hazards and/or failure points identification.

Those who share in it are: people with variety of expertise such as operations, maintenance, instrumentation, engineering/process design, and other specialists as may be required. These should not be “newbies,” but people with experience, knowledge, and an understanding of their part of the system.

Friend, HAZARD & OPERABILITY (HAZOP) study is conducted based on BS IEC 61882 guide. The leader should be a trained and qualified leader in HAZOP, experienced in process (minimum experience of 5 years, as per industry best practices) and the team comprises maily of experienced Engineers in the disciplines of Process / Production, Process Safety, Mechanical, Instrumentation, Electrical and Project. Sometimes Speciailists and plant operators will be asked to participate as and when required. The study will be recored by a Scribe. The study extensively follows EFD or PID and design documents. Once study completed, the leader will publish it to the management with recommendations / action items after conducting a closeout meeting with the area risk manager.

The report is not shared publicly and not allowed as per employment contracts.

The following website could be useful to understand about this.

There are many software in market such as LEADER.

Wish you all the best.
