I do believe that, the main obstacle arises when the employees are "out of concentration " to their job duties.

I think reasons are time specific such as when salary revision is due , most of the employees will be pre-occupied with the thought of how much they are going to get or how much more or less their peers will be getting etc. which I would say is a human tendency. This very thought distracts the employees from concentrating on their job.
To avoid rules & regulation's of company distracts work hour.
