Salaried employees are paid a fixed amount to do the job.  They are not paid by the hour.  Usually the salary is an annual amount, paid monthly in 12 equal installments.   The word comes from the Latin salarium, originally the amount allowed to soldiers for the purchase of salt i.e. their pay.  This does not change because one day's march took longer than usual, or go down if the march was quick.

Some people who are paid for the number of hours worked receive their pay monthly.  Technically, this is not a salary since it varies each month.

Of course, it all depends on the contract.

Yes, usually an employer is entitled for 40 hours per week, 8 per day, anything extra the employee should be compensated by overtime pay.
