1. lack of taking ownership to resolve a client issue or concern
2. getting different answers from different people for the same question
3. not being well informed with escalation processes

Long wait, having my call droped if line is busy (not being able to wait), annoying music while waiting, not knowing how far in line I am, endless IVR, not being able to reach a living person, being transferred several times, having my line dropped while waiting or while being transferred, and THEN, when I have FINALLY reached an actual person, not having my problem solved.
