How do we get a negative bank balance in a Bank Book?

" It should be ensured that the balance in the savings account does not turn into negative balance solely on account of levy of charges for non-maintenance of minimum balance"

That may be due to numeric error while crediting bank leger or not recording debit enteries

Bank OD is there , then we will get a negative bank balance in a bank book..
