Proposal form used almost every type of insurance . It is eventually designed to provide the insurer with full details of the risk , when proposal form completes it becomes the basis of which contract of insurance is based .

 The proposal form end with the declaration that " To the best of the proposer's knowledge and belief the answers true and complete  " .

 This mean the insured and insurer has knowledge of facts on the basis of which the insurance underwriter needs to assess and evaluate the risk and fix premium accordingly. It is also the essence of Utmost Good Faith insurance principle .
