If you reach it than it's not lean manufacturing, it's all about continuous improvement.
Plan, Do, Check and Act; it's a continuous loop.
When all members of the organization are adopting this loop and convinced that it's the key than they are ideally implementing lean.

You cannot ever say this. It is part of a CONTINUAL process of improvement and you should plan, do, check, act and maintain self criticism to drive improvement.

You can never say you have reached the top in lean manufacturing. That's because lean is about continuous improvement which is the principal focus. By definition therefore, since there is always scope to do better, you can never reach the top.

there is no best in lean thinking ..
when you start thinking Lean you now that there is alowes better than the current because the lean thinkink is continous and end less it is a continous improvement you have to think how to improve your process
