A phone interview is the hardest, because all you can give the other side is a voice, but that doesn't mean you can't succeed in it.
Things to make sure you do, (but keep in mind that a phone interview and doing these things doesn't gurantee you getting the job or a next interview, but sure boost your chances!
1- Be in a very quiet place, because otherwise annoys the employer on the other side.
2- Don't eat or chew gum during the interview (The employer would for sure think that you are not taking this seriously).
3- Make sure you are not busy during that time, and nobody will interrupt you (you need to stay focused)
4- Make sure you do your homework before the phone interview, Study the company, the products, the customers, the latest news, and the employer if you know her/his name ahead of time. (it will show them that you are serious about this job, and you really want it).
5- Be comfortable, and be yourself. Make sure you smile (the tone of the voice changes a bit when you smile. So, they can tell when you are smiling and confident)
6- Make a connection with the employer on the other side.. find something he/she is interested in, or you both have in commen and connect (just like when you make a new friend).. make them like you, and that will dafenetily take you to the top of the list of people to hire, because they want someone they like & knows how to talk with people to work with them.
7- Be confident with what you say, and if you don't know, just say, "I don't know!"
8- Say the truth, and don't try to lie (because that will go against you).
9- Prepare smart questions to ask the employer at the end (They look for that, and if you fail to ask, that might cost you the job you are applying for, and may not be considered for it).
Questions shouldn't be general like something you could easily find online, but they could be something like, "Tell me about a typical day for an employee at this job?"
"What are the qualifications that an employee needs to have to be successful at this job?"
"What is the turnout rate for the company in the last3 years? or for people at this job?"
"How long have you beein working for the company?" (The longer, the better)
WARNING: If this is your first interview with the company, NEVER ask about payment, times off, bonus, or anything like that, UNLESS they start talking about it, then you can ask.
10- And finally, make sure you thank them for their time at the end of the interview, and send them a thank you note by mail, or email soon after that. << they love that, too in a candidate.

In sample answer , Always try to listen first before you answer and when you answer be sure for the data you will say it and be direct to the answer and not turn around and around , finally if you don't know the answer you MUST said sorry i really didn't go through that point , finally Just be yourself and accurate in any data.
