Failure to communicate has a drastic impact on the performance, end results and over all good relationship between the employees. The reasons for such problem could be personal/professional or even non intentional. However, if we stick to the6 C-s of management we can have a nice working environment: Complete, Clear, Correct, Consise, Consideration and Courtesy.

Through regular all hands meetings such issues can be identified and corrective actions taken.
I see that feedback is the main source for identifying communications barriers. It confirms whether receivers did receive what you meant to send or not and why. Overcoming identified barriers depends on the barriers themselves. They could e the language, the media, clarity of the message, the forum, timing, culture difference, etc. Each one of those have ways to overcome.
Effective communication is receptive in nature which generates the desired result / objective.
Effective communication has to be simple to be understood easily. It has to be interesting and logical. I feel that power of knowledge, exposure and experience widens the thought process which enables to communicate effective relevant points / message logically.
Strong listening skill also plays an important role in providing room and winning others to be receptive when you communicate.

If it is Internal Communication, the best way to decide which style, time or means of communication is effective is in consultation with employees & partners themselves. They may choose e-mails or company intra-net postings above meetings or vice versa.
If it is External Communication, there is no harm in getting frequent feedback on your communication outlets & messages, through feedback-surveys aimed at customers & external stake holders. Monitor, review & improve the effectiveness of your communication regularly.
