How did you manage your performance appraisal for keeping in right track?

In my previous jobs/appraisals with IBM, everything went smooth as much as I can tell. 

Part of that was due to the CKARITY and fairness of the appraisal process itself as conducted within IBM, and the other reason was due to the PROFESSIONALISM of the managers who conducted the appraisals !
