Change management is a discipline or a practice, not a function or a department within an organization structure.
Hence, there is no such thing as "Head of Change Management" or "Change Management department" whose manager would have the key responsibility for change management.
Change management needs to be endorsed and embraced at the top management level. and middle-management needs to be empowered to induce change into the organization and into the business model details. Employees need to be encouraged/incented/rewarded to participate effectively in suggesting and executing productive and useful change.

It depends on the size of the company and the size of the change. In some cases, a company apponts a change management VP. In others, the responsibility lies with the PM.
Ultimately, I see that the PM is accountable for the success of the project and that includes change management even if it is managed by another party.
The change is being authorized by board of directors and implemented by CEO and supported by Presidents of the company
