In my opinion, If you feel more stress & loosing your concentration from your work then its time to take some rest.
fatigue, loss of concentration, loss of interest, stress, muscle ache and depression.

When you are unable to think out of the BOX and continuesly forgetting your daily tasks, then it is a time be off from work for a short period to relax with family.
Any time off must be inline with business conditions.
