million dollar question! Agree.
I think its good to take responsibility for certainly anything "under your watch" too easy for management (at all levels) nowadays to point the finger to hide their own failings.
Theres a saying that a manager is only as strong as the weakest player.

It’s easier to blame than admit you’re not that good.
It’s easier to blame than it is to improve.
It’s easier to blame than face your own reality.
We all dealt with people who blame others all the time and who try to be right by making others wrong.
Such people are usually afraid to admit that they share a part of the responsibility and so feel more comfortable when someone else carries the blame for a bad thing that happened to them.
Sometimes the act of blaming others can be a cry of pain and a request for support.
whether one agrees or disagrees, this personality trait manifests itself according to the situation one is in
