This all depends on the learner. The quicker he/ she understands and shows competence, the more coaching on different topics can take place. The coach will learn soon how quickly the learner grasps the content of the coaching.

There's no exact formula , but as a common practise the coach should add gradually  the issues in rapport with the coachee progress.

Yes, I support the focus on the session itself and not on the subject because the session axes several subjects must be fully exploited
