In addition to what mentioned above, there is an effective tool used to identify risks, it is Control Self Assessment (CSA).

Also environmental scanning and SWOT anaysis.

Risk analysis and management, primary and secondary research, through planning and evaluation, consultations, discussions, reviewing strategies before implementation. In order to identify risks within projects, activities, tasks etc... You have to ensure you have a clear understanding of the background/history of the task ahead. Study every aspect of it from different angles, think big and small even to the smallest details. Think of scenarios and evaluate them, think of possibilities or occurrences.
