As a teacher I'll try my best to sort out the issue to facilitate him back to a good performance.
Counsel him/her and try to find out the cause, but do so in a sensitive way. If it is something you can help with, all well and good, hopefully you will be able to move forward. Should the problem be beyond your remit or area of expertise offer to try and find someone who can help and who has knowledge of the particular "issue" that is causing the problem. It is important to get the youngster "back on line" as quickly as possible.
