It was happened in1984. When I solved a problem for Accounting I have given the answer as "400" instead of "400000". (I forgot to add '000's which was marked on top of the question-as in '000s.). I could score only10 marks instead of30 for the correct answer and also lost my University Rank.
Since then I was very careful and not happened to make mistakes out of neglect-ion.
1-. I can handle it myself
It is depend upon what level of authority I am on.
When I am a junior staff, I would definitely highlight it to my seniors for help and resolution. When I am on a senior management role and I made any serious mistake, I have to face the consequences and need myself to take prompt initiatives to ensure least damage to the organisation due to that error or mistake.

I believe everyone make a mistake, afterall we are all humans. There is no shame of asking your seniors or colleagues if you made a mistake and unable to resolve it.
