Mr. Krishnamoorthy has covered most of the points, so I fully agree with him..
I'd like to add the absence of a strategic vision and the very loose laissez-faire management sytle.
I'll add old fashioned traditions. The reason is fear of change.
Complacency at the top management and taking people for granted. Any organization that wants to have a sustainable business model running it is imperative that they listen to the people and have them in a consistantly motivated state to perform.
There are many things but few i would like to highlight as most of respected people above did as well.
