There is a saying " If you can not measure it, you can not improve it" H& Safety performance should be monitored.
There are basic two broad categories:1.
Active monitoring (Pro active) a.
No and Quality of Risk assessments covering work activities b.
Safety training schedule vs actual c.
HS committee meetings schedule vs actual d.
Work place inspections schedule vs actual e.
Safety review meeting schedule vs actual f.
Bench marking with other2.
Reactive monitoring (using incident/ accident data as indicators) a.
Accident/ illness data b.
LTI / Accident rates c.
Civil claims

Others use Reactive indicators, though its not a good bench to mesure performance. For example assesing how many accidents or LTI or fatalities have occured in a year can tell if you are implimenting saftey policies. This is the worst indicator thought the companies  at the pathological stage use alot.
