May be because of some personal problems/issues away from the school, have a private discussion, if required give him guidance and counseling over any issues whether it is with regard to learning or personal. If it is even  more serious, contact the parents urgent; if parents are the cause discuss the matter in an appropriate forum to make sure that the problem is fully resolved. All the approaches should be gentle diplomatic and with utmost care, if required seek the help of experts. Two weeks is a prolonged period for a child to become normalcy in his/her studies.
