The best decision I made was to adopt/embrace an internal performance measurement/improvement system based on actual results vs expected/desired ones. The system depends on correct, accurate and honest assessment of reported/measured results with proper checking, verification and validation of the reported results (input), simply because even with the best system in place, garbage-in would only lead to garbage-out !
