The manager is not safe even though such a system doesn't exist. If such a direct option is there a particular employee may get only a limited number of chances to report such instances--bounce back!!; But I think such a system do exist. The chance of reporting may be just5 to10 % at the most.......It's really a sorry to tell affair and shocking also...........many escape just for want of evidences in support to prove.

Yes - We have a disciplinary committee who deals with such issues.
The Auditor and the legal officer with the members of the committe delibearte on the issue to identify the flaws with sufficient evidence, than the manager is formally charged and terminated or caution depending on the seriousness of the issue.
We have the public service general orders to use as a guide.
Our company has conducted it for one time > it was great experience and I wish if they made it on fixed basis.
