Your question "

How to treat an acute pulmonary embolism with three manifestations of the disease?

Do not understand which3 manifestations you are specifically talking about, but the treament of acute PE in general includes:

1. Heparin for all

2. If massive, urgent embolectomy/endaretrectomy, CDTT (catheter directed thrombolytic therapy), or IVTT (intravenous thrombolytic therapy), whichever is appropriate and avialable. Angiojet and thromboaspiration, and mechanical clot breaking are variations on CDTT.

3. Supportive treatment (IV fluids, Oxygen, Dobutamine, etc.) on merits.
