I still believe that honest communication is the best cure. Instead of finding ways to cover it up, it's best to openly and humbly address it.

give more training and double checking .
Best tool to control mistakes at work is not doing mistake. However, still there are human errors which i believe can be resolve by proper double check audit by supervisors and managers.
If i had done the step of work, i have to make another check that's will guarantee the results, i think this efficient

- Once I understand the task in hand, I would do analysis on possible outcomes and its consequences.
