first we must calm this pain with massage and pysical therapy (electrotherapy) , also we need to stretch muscles and strength the weak muscles
stretching for tight muscles , strength for weak muscles with advice to change life style plus the physical therapy modalities (electrotherapy)
ensure bed is firm rather than saggy, paraspinal muscles release and gentle spinal segment mobilisations.
Pt must be kept in positions of pain relief-supported knee flexion when supine, recovery position if side lying
Lower limb bed exercises will generally ellicit some accesory spinal mvts, which should alleviate back pain

First we have to do assessment, and if the pain due to immobility then we need mobility, active movement, manual therapy techniques to decrease the pain and encourage the movement.. patient and family education..
Back pain treatment due to immobility is: change the positioning in the bed from time to another and to do general lower limb exercises 2 to 3 times per day ( stretching the back lower limb muscles )
The best treatment would be prevention through positioning and regular passive turning.in the case that is has developed, spinal mobilisations over affected area, myofascial release and heat therapy. active stretching and strengthening of weak mucles should begin as soon as is tolerable by the patient.
