Investigating an equipment accident at the workplace.
1. FACT FINDING.
. Done by preserving the scene.
. Identifying witness and persons invilved.
. Collection of scene evidence, pictures markings sketches.
. Collecting CCTV footage.
2. FACT COLLECTION.
. interviews of persons involved and witnesses.
. Verification of information collected and counterchecking.
3. INVESTIGATION;
. Taproot investigation tool used which looks into aspects of person involved e.g. Experience, training and refresher, fatigue levels, human performance issues, driving licences and Airside driving Permits
. The equipment was also investigated on serviceability and adheherence to maintance issues, preuse checks, previous faults noted and ergonomic issues.
. The environment was then looked at in terms of conditions e.g. Dark, rainy, hot and any other factors that could have contributed to the accident.
. Administrative/organisational culture also viseted at this level.
. A flow of events up to when the accident occured is then done to identify causal factors is done. this identifies failures,hazards during the process.
. Also causal factors identified were analysed for root cause and corrective action.
. The corrective actions were communicated and discussed with the managers controlling the area and a corrective action plan discussed and approved by what is to be done by who and by when.
. The report was shared with top management and distributed through work notices for workers to be notified on what happened and discuss in tool box meetings.
