Good leadership includes the ability of managing a group of employees which owns the right amount of autonomy to get the job done. Autonomy is a driving force behind a lot of factors that lead to succes, such as motivation, group dynamism, creativity, enthusiasm etc. I don't think a leader is the responsible one because there was a lack of control, but because there was not enough involvement, communication, as well as a lack of constructing a context where intrinsic motivation is stimulated.
From responsibility point of view, this is fault for both leader and employees, as it shows lack of professional commitment, sincerity and accountability. However at the end of the day, the leader has to take the responsibility for this failure, lack of effort apparently means that there might have been inefficient monitoring, weak supervision as well as lack of drive to make it happen.
Project Success is the responsibility of Project Manager (Leader). To address the lack of effort, motivational techniques should be applied also there is a theory of Carrot and Stick, that can be used.
A good Project manager keeps an eye on all the aspects of the project in order to achieve success, and only project manager has the authority to apply organizational resources in the best possible manner.
