*Near miss is when an accident is missed by chance, therefore investigations should be made immediately to prevent future accidents.

*Unsafe conditions is when the working environment is dangerous and this should be known and documented since the Project Charter. Preventive actions are taken into considerations.

*Unsafe Act is when a dangerous act is made by a non-professional employee or when an SHE agent is not supervising, and that could make a working environment unsafe for all the employees around.

Unsafe acts/behaviors: these are actions taken/not taken that directly caused or contributed to the accident. Unsafe acts are actions performed by someone without due regard for personal safety, that of colleagues or other persons in the vicinity. These actions are usually contrary to rules and regulations, or accepted standard practices and procedures. They are illegal acts. E.g. Failure to wear PPE, horse play, unsafe speed, wearing unsafe clothing, moving dangerous objects unnecessarily etc.

unsafe conditions: Objects and physical states that directly caused or contributed to the accident. Examples are oil spill on the floor, defective equipment etc.

Near miss: is when an uncontrolled activity with tendencies to cause harm or with hazardous histroy is ignored , thereby alrming or reocure  , therefore investigations should be made immediately to prevent future accidents

The Different between near miss, unsafe act and unsafe condition are, the near miss is an unplanned event that did not result in injury, Illness or damage but had the potential to do so while the unsafe act is a task or activity that is conducted in a manner that may threaten the health or safety of  workers and while the unsafe act is an unsatisfactory physical condition existing in a workplace environment immediately before an accident that was significant in initiating the event.

In brief, a near-miss incident could occur in safe condition, could be an ongoing acts or practices that will lead to an incident, an unsafe condition is familiar location or prospect that risks are clear and mitigation measures is needed prior approaching it. unsafe act however; can be an act for a person who become familiar with some type of risks without understanding the possible damage which may lead eventually to an incident.
