With my little experience, Performance appraisal is day to day routine.
I involve all management (from junior to senior managers) to evaluate on a daily, weekly, monthly basis. the six months term finds that everyting is done and ready.
Actually, as the HR person, I act as the support to the management in implementing the system and providing Tools to do this.
I also supervise to make sure that the process is properly conducted.
