Few major reasons for decline in Employee Performance are
1. Workplace Environment (unsafe workplace, unpleasant office atmosphere, lack of facilities, low branding)
2. People: (Bad Managers, unhealthy competition, worng hires, wrong attitudes, no job satisfaction, faulty work assignment, management style)
3. Remuneration: (Low/average salary, lack of employee engagement, rewards & recognition, absence or lack of employee benefits scheme, lack of carreer advancement/training/succession)
There can be multiple reasons under these 3 headings depending upon the sector, financial situation and brand of the company.

Nevertheless, I would like to add that one of the main reasons might also be less interaction with the employees manager which leads to an unstable bond.. and that is easily broken these days.
hi a bad feeling in work place: conflict,no vision for his carrer, bad condition........
or family probléme and he cant balance between work and family .
1. Unachievable targets resulting in stress and demotivation.
2. Insufficient rewards - Intrinsic and Extrinsic.
3. Conflict of Interest.
All factors answered are may be the one which cause decline in their performance. But actually these are minor one.
The major one is the environment of organization and working routine. Employees get bored working in the same way daily and due to this their performance gadually strats declining.
Keep changing the environment and working routines for more productivity.
1-Low pay
2-No appreciation for performance
3-No upgrade/promotion
4- Company values are not matching with his personal ethical belief.
No authority
Issues in delegation of rights
lack of appreciation
refusal on new ideas from him/her

* May be there is no worker inside an Employee.
* May be no Manager(s) inside the organisation.
* May be no Employer for the Organisation.
* May be illiterates at varied levels in the hierarchy.
* May be there is no Carrot or Stick in the Organisation's garden.
