I think only the companies that do not comply with the law that do not provide health insurance. Unfortunately, startup companies may exploit the employees need for a job and money to work without this right.

Well; there is no common practice among startup companies at employee benefits area. So you will find many startups will care to provide its staff with good benefits. Also, many others will take the approach of cutting from employee rights.
i think it is not nessecary at the beggining but you must provide your employees an extra fees in return .you have to make them belive that they are one of the family , increase the boundries between you and them in the begininng
