you have to consider all. but you have to weight quality as the highest of them all. quality reflects your efficiency over effectiveness alone.  it is always an added value to work and company as a whole. it does not only increase sales, but also retention of good customers as an example.

For the sake of transparency yes, because  it is a the tradeline of how your work or application is effective towards your target, or if the scenario isnt that good atleast your have monitored what has/have to be improved.
