I am sure it happened to many a message was passed to you and due to various circumstances it reaches through many hands and when you understand it the source , he meant something else and you have done some other work which was not meant. Did it happen to you ? if yes please share it

To some extent I agree. However - if the communication carries clarity in substance and goal to be achieved. In structured administration, the routing of communication is done professionally; each stage/hand is required to put in comments relevant to the proposed idea, event or product.
Naturally yes, but it also depends on the means of communication being used. e.g. if there is a written communication like emails no matter how many hands it will pass the actual meaning will not change. This however is highly recommended that the end responsible for action must make sure by contacting the initiator of the communication facts to make sure that what he understood is correct and clear. We must not remain in the illusion that communication is made unless we make sure what was intended to be communicated is clearly understood by the end responsible.
The recipient recieves wrong information or misunderstands it when it has only been surbotaged or tempered with through the hands that touch it or access it & the original message is altered. But if the hands that touch it or access it don't surbotage or alter the message, then the recipient will absolutely recieve the right information sent to them through the original message.
I agree in case there is no system controlling the communications, and every one received the message need just to get out of it and not do what he have to do in professional way.
This is the drawback of the system and people who are using it....For example :
If a communication is meant for someone like Scientist and it reaches to a professor of a Sociology , I am not sure how he will interpret it...... Message it meant for the person to whom it is addressed, the sender knows the receiver and he can communicate accordingly.
