Following are the reasons and these are often named as3C: -

1. Competing Demands of work: To meet production targets people may violate set safety procedures.

2. Complacency: Due to complacency workers feel confident and on the basis of their experience they show complacent behavior towards set safety procedures.

3. Complexity of Safety Procedures: Sometimes the complexity of the procedures makes it difficult to act upon them.
