The best way to understad the process behind it is For example, suppose that onea dministrator moved a user into the Sales organizational unit (OU), at the same time another administrator deleted the Sales OU on another domain controller. When convergence occurs, where does the user account wind up? In Lost and Found Folder. The bottom line is when multiple administrators land up performing all most similar action on same object or attributes which results in a conflict and makes it to appear  in Lost and Found Folder.
